Patient-Centered Medical Home
Patient-centered medical homes strive to increase patient engagement and enhance coordinated care to improve quality and eliminate waste. PCMHs pursue these goals by meeting a set of performance guidelines such as enhanced access and continuity, providing self-care and community support resources, and improved performance overall.
In order to ensure success as a PCMH, there is a need for a strong EHR partner that supports the goals of patient engagement, coordinated care, interoperability and quality outcomes.

The National Committee for Quality Assurance (NCQA) works with clinics to help them become patient-centered medical homes focusing on building partnerships between individual patients, their personal physicians, and if appropriate, the patient’s family. Quality healthcare is enhanced through registries, information technology and health information exchange (HIE).
Health centers can apply for PCMH recognition through NCQA and qualify for increased reimbursement. Additional benefits gained from participation in this program include a clearly-defined process in order to become a PCMH, and the criteria are available at no cost on the NCQA website.
