Creating a Personal Health Record with HealthVault and PrimePATIENT
Microsoft HealthVault is a free service within the Greenway PrimeSUITE secure web portal that enables individuals to take a more active role in their own care by creating, updating and sharing their health history in their own personal health record (PHR).

The seamless integration of the PrimeSUITE® electronic health record, its password-protected PrimePATIENT® online portal and Microsoft HealthVault creates a single, convenient and sharable source of personal health data — information that dramatically improves care coordination across all of the person’s caregivers, authorized family members and other support persons.
Getting started
To create their own HealthVault account, patients of physicians using PrimeSUITE simply access a signup link found on the PrimePATIENT portal and integrate the portal’s digital health history forms into their new HealthVault account. Going forward, participants can easily go online to update their personal health record information, and data can be uploaded directly from a number of compatible home-health medical devices such as blood pressure or glucose monitors.
Following each office visit, a person’s caregivers can also import entire PrimeSUITE clinical summaries or chosen elements into their HealthVault record through a standards-based Continuity of Care Document (CCD) to create a complete and timely medical picture for each person.
Sharing information
With HealthVault, persons remain in control of what information they choose to share with family members or their other healthcare providers, such as specialists and Emergency Department personnel.
For authorized family members helping manage or monitor the care of a loved one such as an aged parent, the combination of HealthVault and PrimePATIENT enables them to easily stay current with the person’s care plan by monitoring vital signs, medications and reactions, immunization schedules or discharge instructions.
