<h2>Greenway customers are ACO and PCMH-ready</h2>
<p>An accountable care organization or accountable care community is defined as a delivery reform model with the overall goal to improve care quality while reducing the total cost of care. It seeks to link provider reimbursements to quality metrics in order to achieve this goal.</p>
<p>The patient-centered medical home (PCMH) is an inventive program that focuses on improving primary care. The certification program is outlined by a set of clear standards, empowering practices with information needed to personalize care to their patients and enabling providers to work in teams to better coordinate care.</p>
